Add "How to Report a Bug" to Help menu

* lisp/info.el (info-emacs-bug): New command.
* lisp/menu-bar.el (menu-bar-help-menu): Add "How to Report a Bug" to Help.
* lisp/mail/emacsbug.el (report-emacs-bug): Replace with info-emacs-bug.
